General Terms and Conditions 

 

1.  By signing the history sheet (electronic form), you have acknowledged all terms and conditions of the clinic.

 

2.  Your personal and medical information is securely stored and not shared with any external agency for marketing purposes.

 

About Personal Information

 

3.  It's the patients/carers/parent's responsibility to provide correct and accurate personal and health-related information about the patient to the best of their knowledge. 

 

4.  For ongoing patients: It's the patients/carers/parents' responsibility to update correct and accurate personal and health-related information about the patient to the best of there knowledge.

 

5.  The practice can use your personal and medical information to train doctors and nurses, and whilst discussing the treatment options with internal or external practitioners such as specialists.

 

6.  From time-to-time practice can send you the clinic's promotional offers.

 

7.  The patient willing to access personal information such as X-rays individual files should do via written request. Once the request is received, it may take up to 7 working days for X-rays and 21 working days for the personal file to be released. Administrative fees ($50) may apply.

 

About the condition of Payment and Payment Plan

 

8.  The patient should pay the treatment fees in full at the end of treatment.

 

9.  It’s the patient’s responsibility to enquire about the cost of treatment with the dentist before the commencement of the treatment.

 

10. Failing to pay for the treatment will be referred to the police or debt collection agency.

 

11. The patient will be charged $50 cancellation fees additional in their upcoming appointment if they fail to attend their appointment or cancel or reschedule an appointment within 48 hrs of their appointment.

 

12. $50 additional fees apply for consultation (30 minutes), treatment plan, treatment quotations, and referral letter.

 

13. 5% Surcharge applies for payment through Denticare payment plan.

 

14. Patients should not assume the clinic's responsibility for Payment plan related disputes or issues as Payment plans are offered by a third-party provider.

 

15. It’s a patient’s responsibility to consider their financial condition or situation before applying for the payment plan.

 

16. Its the patient’s responsibility to read and understand the terms and conditions of the payment plans offered by external providers such as DentiCare, ZIP Money and AfterPay. 

 

17. Patients using payment plan options have to resolve payment-related disputes such as failure to pay or extension for payment by raising directly with the appropriate payment plan providers. 

 

18. The clinic is not liable for any financial difficulty the patient may faced which is directly or indirectly related to payment plans.

 

19. It’s the patient’s responsibility to inform the payment plan provider where the patient decide to cease or amend ongoing treatment.

 

About Appointments

 

20. It's a patient's responsibility to maintain their appointment, especially patients with a more extended treatment plan.

 

21. Patient with hospital voucher: If the patient fails to attend the appointment regularly, the clinic will send the voucher back to the Hospital. The practice is not responsible for applying the extension of the hospital vouchers.

 

22. It's the patient's responsibility to maintain the appointment, especially undergoing treatment that requires multiple appointments. The clinic is not liable for the failure of the treatment as a result of failing to keep the appointment.

 

23. The patient will be charged non refundable $100 deposit fees for the booking of long duration appointment ( For example: Whitening, Crown, Implant, RCT appointments). This fee will be used as cancellation fees if the patient failed to attended or reschedule their appointment within 48 hrs of scheduled appointment. For the patient who keeps the appointment, deposit will be used towards their cost of treatment.

 

24. The patient will be charged 50% deposit for treatment involving external lab or agencies. If the patient has change of mind for the treatment, the lab cost and any other associated costs will be deducted from the deposit.

 

Other Important Stuff

 

25. The dentist has all right to refuse the treatment if the patient is deemed disrespectful, aggressive and abusive, unreasonable or obnoxious towards staff or doctors.

 

26. The clinic does not intend to in any way or form encourages patients to make their treatment decisions based on financial gain, primarily through promotional or other offers.

 

27. It's the patient's responsibility to inform the doctor if they wish to have a second opinion.

 

28. It’s the patient’s responsibility to maintain their dental hygiene, make regular appointments for a check-up, clean, and follow the instructions provided by the practitioner.

 

29. The above terms and conditions are subject to change and update from time to time without notice.

Birthday 20% discount gift-Terms and Conditions

  1. The following terms and conditions are in conjunction with the ‘General Terms and Conditions’
  2. The offer is valid until the next birthday.
  3. The offer can be redeemed only once before expiration.
  4. The offer is valid for only fully paid new treatments.
  5. The patient cannot redeem the offer with any other current offers.
  6. Offer cannot be redeemed for cash.
  7. Offer cannot be redeemed for any ongoing treatments, where multiple treatments and appointments are required.
  8. The offer is not transferrable.
  9. Offer is not valid for Implant and Orthodontic treatments.
  10. Offer is not valid for any cost of quotations, treatment plan, x-rays, OPG or referral.
  11. Offer is not valid for Child Dental Benefits Schedule, No GAP Scale and clean (Offer for Private Health Fund Members), Hospital Voucher Patients, DVA patients, and any other current offers run by the clinic.
  12. By accepting the offer and treatment, you acknowledge all terms and conditions of the clinic.
  13. The clinic does not intend to in any way or form to encourage patients to take their treatment decisions based on financial gain. The offer should be solely considered a token of the clinic's gratitude.
  14. It’s the patient’s responsibility to inform the doctor if they wish to have a second opinion.
  15. The patient should pay the payment of the provided treatment in full at the end of treatment.
  16. It’s the patient’s responsibility to enquire about the cost of treatment with the dentist beforehand.

Initial/Complementary Consultation - Terms and Conditions

  1. The following terms and conditions are in conjunction with the ‘General Terms and Conditions’
  2. Initial Consultation / First consultation is complimentary for only new patients who booked online. Please read the following "Terms and Conditions" carefully.
  3. The complimentary consultation is for 15 minutes.
  4. Charges may apply if the complimentary consultation lasted for more than 15 Minutes.
  5. Fees apply for any intraoral X-rays or any treatment done during the consultation. It's the patient’s responsibility to inform the doctors if they are unwilling to undertake X-rays or treatment during the consultation.
  6. Fees apply for any referrals or quotations of the treatment. (If requested by the patient)
  7. At the end of their appointment, the patient needs to pay any fees accrued for X-rays, referrals, quotation for treatment, or any treatment done.
  8. Patients with the Private Health Fund may be charged No GAP consultation fees.
  9. Complimentary Initial / First Consultation is not valid to Child Dental Benefit Scheme - CDBS and Hospital voucher patients.

Scale and Clean Offer - Terms and Conditions

 

The following Terms and Condition is in conjunction with General Terms and Conditions.

  1. Offer is valid for the limited time only.
  2. Offer is valid for the patient who has maintain their regular dental hygiene that is scale and clean and regular check-up with the dentist.
  3. Secondary scale and clean at an additional cost will apply with the patient with poor dental hygiene. For patient with Private Health Fund (PHF) GAP payment will apply for secondary scale and clean.
  4. The offer includes 2 X-rays, Scale and fluoride treatment. Additional cost applies for other treatments or procedures such as additional X-rays, OPG, etc. For Private Health Fund patients, GAP payment applies on additional procedures or treatments.
  5. This offer cannot be used in conjunction with other offers.
  6. Due to strict guidelines from Private Health Fund Companies, Private Health fund members must produce their health fund card for processing, patient need to pay in full if failed to produce PHF member card.
  7. It’s the PHF members responsibility to check their validity and coverage of their insurance. PHF members with invalid or with no dental coverage must pay in full for the provided treatment.

ZOOM Teeth Whitening – Terms and Conditions

  1. The following Terms and Condition is in conjunction with General Terms and Conditions.
  2. This offer includes Scale and Clean, in chair teeth whitening, Custom-made take-home whitening tray/Kit.
  3. Offer is valid for the limited time only.
  4. Offer is valid for the patient who has maintain their regular dental hygiene that is scale and clean and regular check-up with the dentist.
  5. Secondary scale and clean at an additional cost will apply with the patient with poor dental hygiene. For patient with Private Health Fund (PHF) GAP payment will apply for secondary scale and clean.
